Care & services

SFA is committed to helping provide the best possible life for the people we support.

We take a family-orientated approach to how we support both individuals and our employees. We put people first because we care.

We don’t just talk about providing individualized supports, we stand behind it. Being a smaller agency we’re able to take the time to get to know each person as an individual and create a plan that meets their unique needs and interests.

From your home to ours, we make the transition easy with our family orientated and caring approach to providing supports.

Anyone interested in accessing adult developmental services must contact Developmental Services Ontario (DSO) at www.dsontario.ca. The DSO completes an application process and determines eligibility. It is recommended that families begin the application process at age 16 to help with the transition to adult services at age 18.

Supported Group Living Homes

SFA operates 13 supported group living homes in three counties (Sarnia-Lambton, Chatham-Kent and Windsor-Essex). We support adults with developmental disabilities to live their best lives.

Our caring employees take a person-centered approach by listening to individuals to ensure the activities and supports they receive are based on their interests and wishes. We also support individuals to develop life and communication skills so they may live life as independently as possible.

Youth Transitioning to Adult Supports

Our Stepping Stones program is a service designed to help youth transition from school to developing connections within their community as an adult. Our Stepping Stones program provides person directed plans, goal setting, and transition planning services to each participant. Services are currently only available in Sarnia/Lambton.

Supported Independent Living

This service is designed to support people with disabilities who are living independently within their community. Support is based on a person's individual needs as identified in their Person Directed Plan.

The goal is to offer people the greatest potential for success in an independent environment. Currently these services are only available in Sarnia/Lambton.

Transfer Payment Agency (TPA) Services

Individualized Supports

SFA acts as a TPA to receive allocated funding provided by the Ministry of Children, Community and Social Services on behalf of the individual and their family. Our employees assist in developing a budget and ensuring funding accountability and quality assurance measures are maintained.

Passport Funding

SFA assists in developing a budget and submits invoices directly to Passport. We will also help by making direct payment of bills to other services and supports, as requested.
